WhatsApp appointment booking for clinics: a time slot by message, no app

WhatsApp appointment booking is a way for a patient to book a named time slot at a clinic by sending a WhatsApp message: they pick a day, pick a free time, say what the visit is for, and get a confirmation with buttons to reschedule or cancel. There is no app to install and no website to log into. How WhatsApp appointment booking works, step by step

The patient scans the QR for that doctor or department, and WhatsApp opens on the clinic's number with "I'd like an appointment" already typed. They tap send. Or they skip the QR and message the clinic in their own words. The first reply is a WhatsApp list of the coming days that still have free slots, each with a count, such as "Sat, 26 Sept · 8 free". Up to ten days are shown, within however far ahead the clinic allows booking.

Tapping a day brings up the free times for that day, eight to a page, with "Later times" and "Other days" at the foot of the list. A patient who would rather type can reply with the option number, "2", instead of tapping. If two patients tap the same slot at the same moment, one of them gets it and the other is told it was just taken and picks again, so a slot is never booked twice.

On a first visit the patient is asked their name, once. Returning patients are booked under the name they gave before. The last question is what the visit is for, and then the confirmation arrives: an appointment number such as D004, the day, the time and the reason, with Reschedule and Cancel buttons underneath.

The visit reason, typed or spoken in the patient's own language

The reason question is where most booking forms lose people, because it asks for typing on a small keyboard in a language the form chose. Here the patient types it or sends a voice note; the question is required, so every booking reaches the doctor with a reason. Tamil, Hindi, Telugu, Kannada, Malayalam, English and the code-mixed messages people actually send are all accepted, typed or spoken.

Doctor appointment booking on WhatsApp: what reception sees

For an appointment department, the queue page becomes the day's appointment book: every slot in time order, booked ones with the patient's name and reason, free ones marked "Free · tap to book". Reception can step through today and the coming days, and the top of the page counts what matters for the session: booked, arrived, done and free slots.

Phone-ins and patients at the desk go into the same book. Reception taps a free slot and enters the name, the WhatsApp number and the reason, and that booking gets the same reminder a patient's own booking would. An older patient who will never message the clinic is booked in the same few seconds as one who did.

Each booked row carries the actions a morning needs: Check in when the patient arrives, Call, then Done, where the fee is noted if the clinic keeps the Collections log. Move reschedules, No-show and Cancel do what they say. "Call next" calls the arrived patient with the earliest slot rather than whoever walked in first, so someone who turns up early for 11:30 does not jump the 11:00 patient. The waiting-room TV calls the appointment number aloud, exactly as it does for a token.

Appointment reminders on WhatsApp, and what happens to a no-show

Before the slot, the patient gets a reminder on WhatsApp with two buttons, "I'll come" and "Cancel". The clinic chooses how long before: 1, 1.5, 2 or 3 hours, with 2 as the default. At any point the patient can reply "cancel" or "reschedule", or send "status" or "hi" to see what they have booked. Rescheduling moves the same booking to a new time; cancelling frees the slot for someone else straight away.

Some patients will still not come. A booked patient who has not checked in 30 minutes after their slot, or two slot lengths after it when that is longer, is marked a no-show automatically and the slot is released. Nobody at the desk has to remember to do it. No-shows are counted in the reports and in the owner's closing summary for the day, which is where a pattern, one session or one weekday that loses more than the others, becomes visible.

Tokens or appointments, chosen per doctor

Every department in TOKENN has one setting that decides how patients join: Token queue or Appointments. Tokens suit a GP or general OPD, a pediatric clinic, any walk-in heavy practice, and any day where consult length swings from two minutes to twenty. Appointments suit visits of a known length: dental procedures, physiotherapy sessions, orthodontic reviews, cosmetic dermatology and laser, ayurveda therapy sessions, and specialists who already run to a diary.

A polyclinic usually wants both, and runs both: the GP on tokens and the dentist on appointments, on the same WhatsApp number and the same account. Setting up a clinic appointment system in India in one sitting

Open the Departments page, pick the department, and set "How patients join" to Appointments. Then three choices: the slot length (5, 10, 15, 20, 30, 45 or 60 minutes), how far ahead patients can book (anything from 1 to 30 days, 7 by default), and how long before the slot the reminder goes out. Slots are cut from that department's appointment hours and closed days, so 09:00 to 17:00 with 15-minute slots gives 32 slots a day. A morning and an evening session on the same day are both supported.

A clinic that runs mostly to appointments can make every new department start as appointments from Settings. Switching a department back to tokens is allowed once it has no booked appointments left, so a trial is easy to undo. Print that department's QR, put it where patients will see it, and the next message it brings is an appointment request rather than a token.

What appointment booking costs

A booking costs ₹1, the same as a token, taken once when the booking is made, whether the patient booked on WhatsApp or reception booked them from the desk. Rescheduling is free, however many times it happens. A cancelled or missed booking is not refunded, which is the same rule tokens follow. Reminders are included, there is no subscription, and credit is bought in prepaid packs that never expire; the packs are on the pricing page.

Two limits keep one phone from filling the book: one booking per phone number per clinic per day, and at most three WhatsApp bookings from one number in any 24 hours. Both exist so that a slot another patient needs is not held by someone booking speculatively.

The boundary: the reason line is a summary, not a diagnosis

The assistant books, reschedules, cancels and shows a patient their booking. The line it writes for the doctor is the patient's own account, shortened, with the original words kept beside it; it is never an assessment of what is wrong. It does not triage, does not suggest a medicine and does not answer medical questions, and whatever the patient said stays theirs to explain when they sit down.

What it is not: no booking website, no payments, no calendar sync

Booking happens on WhatsApp or at the desk and nowhere else. There is no web booking page or widget to embed, no online payment or deposit (the fee is collected at the clinic as it is today), and no sync with Google Calendar. Each slot holds one patient, and there are no group slots.

It also does not send messages the patient did not book: there is no day-before reminder, no "doctor running late" alert, and no recall list chasing people who are due back. It is not a video consultation tool and holds no medical records. What it does is the booking itself, the reminder before it, and the day book the desk runs from.

A WhatsApp appointment bot instead of the phone at reception

Most clinics that ask for appointment software are really asking to stop answering the phone during consults. The calls are the same four every day: is there a slot, can I come later, I cannot make it, what time was mine.
